About AEGIS

Antimicrobial & Epidemiological Guidance Intelligence System

Overview

AEGIS is a comprehensive healthcare intelligence platform designed to support Antimicrobial Stewardship Programs (ASP) and Infection Prevention (IP) teams. The system integrates clinical data to provide real-time surveillance, decision support, and regulatory reporting capabilities.

By combining automated detection algorithms with clinical workflow tools, AEGIS helps healthcare teams identify opportunities for intervention, track outcomes, and maintain compliance with quality reporting requirements.

Modules

📞

ABX Approvals

Phone-based antibiotic approval workflow for pharmacists handling prospective review requests. Provides clinical context including MDR history, drug allergies, and renal function. Features approval duration tracking with automatic re-approval requests when patients remain on antibiotics past the approved period, and approval chain tracking across re-approvals.

💉

Dosing Verification

Real-time verification of antimicrobial dosing, interval, and route based on clinical indication, patient factors, and drug interactions. Evaluates dose appropriateness using indication-specific rules (e.g., CNS penetration for meningitis), renal adjustments, weight-based calculations, age-specific dosing, route verification (e.g., PO vancomycin for C. diff), drug-drug interactions, allergy cross-reactivity, and duration appropriateness. Generates tiered alerts for critical issues (wrong route), high priority (no renal adjustment), and moderate optimization opportunities (extended infusion candidates).

ASP Alerts

Real-time alerting for blood culture results and antimicrobial therapy reviews. Identifies patients with positive cultures requiring stewardship attention, tracks therapy coverage, and facilitates timely interventions. Supports acknowledgment workflows and resolution tracking.

🏥

HAI Detection

Automated surveillance for all 5 healthcare-associated infection types using NHSN criteria: CLABSI, SSI, CAUTI, VAE, and CDI. Combines rule-based screening with LLM fact extraction and deterministic NHSN rules to identify candidates. Supports IP review and override workflows with FHIR-preferred data sourcing.

💊

Antibiotic Indications

LLM-based indication extraction and antibiotic appropriateness assessment. Extracts clinical syndromes from notes and evaluates whether prescribed antibiotics align with local guidelines. Identifies febrile neutropenia patients and supports appropriateness reviews.

📋

Guideline Adherence

Population-level monitoring of clinical guideline and care bundle compliance. Tracks adherence to evidence-based bundles (e.g., sepsis, surgical site infection prevention) at the element level. Generates alerts for non-adherence and supports quality reporting.

🩺

Surgical Prophylaxis

Perioperative antibiotic validation for surgical patients. Monitors timing of prophylactic antibiotics relative to incision, validates agent selection based on procedure type, and tracks duration to prevent excessive post-operative dosing.

🧬

Drug-Bug Mismatch

Real-time detection of therapy-organism mismatches based on culture and susceptibility results. Identifies coverage gaps where current antibiotics may not be effective against isolated organisms, and opportunities for de-escalation to narrower-spectrum agents.

🦠

MDRO Surveillance

Multi-drug resistant organism tracking based on culture susceptibility results. Monitors MRSA, VRE, CRE, ESBL, CRPA, and CRAB across the institution with unit-level dashboards and trend analysis for infection prevention teams.

🚨

Outbreak Detection

Cluster detection and investigation across MDRO, HAI, and CDI cases. Uses spatiotemporal clustering algorithms to identify potential outbreaks and supports IP investigation workflows with timeline and contact tracing views.

📊

NHSN Reporting

Antimicrobial usage (AU) and antimicrobial resistance (AR) reporting for the CDC's National Healthcare Safety Network. Aggregates usage data by drug and location, tracks resistance patterns, calculates standardized metrics, and generates CDA files for electronic submission.

📊

Action Analytics

Unified tracking and analysis of all ASP and Infection Prevention actions across modules. Provides recommendation breakdowns, approval workflow analytics, therapy change tracking, unit-level comparisons, and time/workload analysis with CSV exports for committee presentations.

📈

Dashboards

Analytics dashboards providing cross-module insights for ASP/IP leadership. Includes unified metrics tracking, provider workload analysis, trending comparisons, and LLM model training progress tracking for continuous improvement.

Technology

AEGIS is built with Python and Flask, using SQLite databases for data persistence. Clinical data is sourced via FHIR R4 APIs (Epic-compatible) with Clarity fallback. AI-assisted clinical extraction uses local Ollama LLMs (llama3.3:70b) to keep all PHI on-premises. The system is designed for institutional deployment with no external AI service dependencies.

Demo Environment: All patient data displayed is simulated. No actual patient data is available through this dashboard.
